Order Picker Fall Protection Harness And Lanyard Inspection

Daily pre-use inspection procedure for full-body harnesses and shock-absorbing lanyards used on order pickers and vertical mast work platforms. Complies with CSA Z259.10, CSA Z259.11, and WorkSafeBC OHS Part 8. This inspection must be performed by the operator before the first use of each shift.

Purpose

The purpose of this Standard Operating Procedure is to establish a consistent, documented inspection routine for fall protection harnesses and lanyards used by order picker operators. The primary objective is to identify wear, damage, chemical degradation, or manufacturing defects that could compromise the integrity of the fall arrest system before it is relied upon at height. A failed inspection must result in immediate removal from service and tagging as condemned. This procedure is the operator's first line of defence against catastrophic fall arrest system failure.

Scope

This SOP applies to all full-body harnesses (CSA Z259.10) and shock-absorbing lanyards (CSA Z259.11) issued for use on order pickers, turret trucks, vertical mast work platforms, and any other elevated material handling equipment where a fall restraint or fall arrest system is required. It covers pre-use inspection only — not periodic advanced inspection (which must be performed by a competent person every 6 months per manufacturer specifications). This document is intended for warehouse operators, order picker drivers, safety coordinators, and shift supervisors in distribution centres, cold storage facilities, and manufacturing warehouses across Canada.

Safety Precautions
  • LOTOEnsure the order picker is parked on level ground, the mast is fully lowered, and the key is removed before inspecting any fall protection equipment near the machine. Do not perform inspection with the platform elevated.
  • TAGAny harness or lanyard with suspect webbing, hardware deformation, broken stitching, or illegible labels must be immediately tagged with a "DO NOT USE — CONDEMNED" tag and removed from the work area. Never leave failed equipment in circulation.
  • NONever attempt to repair, sew, re-weave, or alter a damaged harness or lanyard. Only the original manufacturer may perform repairs. Any equipment that fails inspection must be cut and discarded to prevent re-use by others.
  • CHEMBe aware that battery acid, hydraulic oil, cleaning solvents, and freezer/cooler condensation can degrade harness webbing invisibly. Any harness exposed to corrosive chemicals must be removed from service and logged for advanced inspection regardless of visual appearance.
Required Tools & Materials
  • Full-body harness — CSA Z259.10 compliant, correctly sized for the operator
  • Shock-absorbing lanyard — CSA Z259.11 compliant, with locking snap hooks on both ends
  • Inspection log or digital checklist (paper log or SafeDesk mobile app)
  • Flashlight or bright task light for shadow-free inspection of webbing and hardware
  • Indelible marker or inspection tag (date/label for pass)
  • Clean, lint-free cotton cloth to wipe away dirt and grease from webbing
  • Gate gauge or set of feeler gauges to check snap hook gate gap (optional but recommended)

Procedure: Step-by-Step
1 Remove harness and lanyard from service and lay flat. Take the harness out of its storage bag or off the hook. Lay the entire harness flat on a clean, well-lit workbench or clean pallet. Place the lanyard alongside it. Do not inspect while hanging — you need full visual and tactile access to both sides of every strap.
2 Inspect webbing — both sides, entire length. Using both hands, slowly work the webbing through your fingers from one end to the other. Apply gentle tension to expose hidden cuts or fraying. Look for: frayed fibres, cut strands, abrasion marks, chemical burns (discoloured or stiff spots), ultraviolet degradation (faded or chalking), and mould or mildew. Check the back side of every strap. Any exposed cut fibres or broken threads — immediately remove from service.
3 Inspect all stitching and stress points. Examine all sewn junctions: sub-pelvic strap connections, shoulder strap crossovers, D-ring pads, and tongue/buckle anchors. Look for pulled or broken threads, missing stitches, or abrasion through the thread. Tug gently on sewn-on pads and labels. If any stitching is broken or pulled, the harness must be tagged out and replaced.
4 Inspect all D-rings and hardware. D-rings (dorsal, and if present, sternal and side) must be free of cracks, burrs, sharp edges, distortion, corrosion, and weld splatter. Test each D-ring by rotating it in its pad — there should be no binding or rough movement. All steel hardware must show no rust pitting or plating loss. Buckles and adjusters: operate each buckle fully — tongue must seat firmly; friction adjusters must hold webbing without slipping. Replace any buckle that does not positively latch.
5 Inspect lanyard webbing and shock absorber pack. Follow the same webbing inspection procedure for the entire lanyard length. Pay special attention to the shock absorber pack: check that the protective sleeve is intact, the stitching on the energy-absorbing element has not been torn or elongated, and there is no evidence of deployment (opened pack, exposed internal webbing, elongated tear strips). If the shock absorber shows any sign of activation, the lanyard is immediately condemned.
6 Inspect snap hooks and carabiners. Each snap hook and carabiner must be checked: gate opens and closes freely without binding. Gate spring returns the gate fully to the closed/locked position with an audible click. Check the gate locking mechanism (twist-lock or double-action) — it must not close by gravity alone. Inspect the nose and keeper for wear, burrs, or deformation. Measure gate opening with a gauge — must not exceed manufacturer specification (typically 0.5 mm wear allowance). Any hook with a bent or worn gate is condemned.
7 Inspect labels and date tags. Locate the manufacturer label on the harness and lanyard. Verify that all required markings are legible: manufacturer name, model number, serial number, date of manufacture, size, and CSA/ANSI standard mark. Confirm the harness has not exceeded its service life (typically 5 years from date of manufacture unless otherwise specified by manufacturer). If labels are illegible or missing — remove from service.
8 Document inspection results and tag equipment. Record the inspection on the official fall protection log or digital checklist (SafeDesk). Include: equipment ID, date, inspector name, result (pass/fail), and any remarks. If the equipment passes: attach a new inspection tag or mark the inspection date on the existing tag. If it fails: tag the equipment with a 'DO NOT USE — CONDEMNED' tag and remove from the work area. Notify the supervisor immediately.
