Purpose

The purpose of this standard operating procedure is to define a repeatable inspection process for mezzanine fall protection gates and personal fall arrest harnesses. Regular inspections identify wear, damage, or malfunction before a fall incident occurs, ensuring compliance with WorkSafeBC's fall protection requirements (OHS Regulation Part 11) and reducing the risk of serious injury or fatality.

Scope

This SOP applies to all mezzanine loading/unloading areas equipped with swing gates, sliding gates, or vertical lift gates, and to any personal fall arrest harnesses used for work at heights above 3 metres (10 ft). It covers visual and mechanical inspection of gate structures, hardware, self‑closing mechanisms, interlocks, and the full harness assembly including webbing, stitching, D‑rings, buckles, and lanyards. This procedure does not cover anchor points or lifeline systems; those require separate engineered inspections.

Safety Precautions
  • LOTOLockout/tagout the mezzanine area before performing any inspection that requires the gate to be opened or removed. Ensure no moving equipment can enter the drop zone during the inspection.
  • PPEWear approved fall protection harness with lanyard attached to a certified anchorage point when working near the mezzanine edge. Also wear safety glasses, gloves, and a hard hat.
  • EXCLExclude unauthorized personnel from the inspection area. Post warning signs at both the mezzanine level and ground level.
  • DO NOTDo not use temporary ladders or platforms to access the mezzanine edge unless they are secured and rated for the load. Always maintain three points of contact.
  • DOCReport any observed safety defects immediately to the supervisor and tag the equipment as "Do Not Use" until repaired or replaced.

  • Fall arrest harness (if not already worn) and shock‑absorbing lanyard
  • Camera or smartphone for photo documentation of defects

Procedure: Step-by-Step
1 Prepare for Inspection. Obtain all required tools and PPE. Review the manufacturer's inspection guidelines for the specific gate and harness model. Verify that the mezzanine area is clear and that LOTO has been applied if the gate will be disabled during the inspection.
2 Inspect Gate Structure and Frame. Using the flashlight and mirror, examine the gate frame, posts, and mounting brackets for signs of deformation, cracks, corrosion, or missing fasteners. Check that all bolts are present and torqued to the value specified in the manual (use torque wrench). Confirm the gate is plumb and square within the opening.
3 Inspect Gate Hardware and Hinges. Open and close the gate fully. Observe the hinges for wear, binding, or excessive play. Lubricate if required (use manufacturer‑recommended lubricant). Verify that hinges are free of rust and that pivot pins are secured with cotter pins or set screws.
4 Test Self‑Closing Mechanism and Latches. Release the gate from a partially open position (about 45°). It must swing or slide to the closed position without human assistance. Check the latch engages fully and the gate cannot be dislodged by a lateral force of 100 N (approx. 22 lbf). If an interlock is present, verify that the interlock blocks the opening when the gate is open, and releases only when the gate is fully closed and latched.
5 Inspect Personal Fall Arrest Harness. Lay the harness flat on a clean surface. Inspect all webbing for cuts, fraying, pulled strands, burns, chemical damage, or UV degradation. Run the webbing through your fingers to detect hidden damage. Check all stitching (load‑bearing seams must be intact, no broken threads). Inspect D‑rings for cracks, distortion, or corrosion. Check buckles and adjusters for freedom of movement and full engagement.
6 Inspect Lanyard and Snap Hooks. Examine the shock‑absorbing lanyard for signs of deployment (deployed pack indicates previous fall arrest). Check the webbing or cable for damage. Ensure snap hooks close fully and the locking mechanism functions correctly. Test the gate keeper (double action).
7 Document Findings. Record the inspection date, inspector name, gate ID/location, harness serial numbers, and any defects found. Use the inspection log sheet. Photograph any defects and attach to the log. If any component fails inspection, tag it with a "Do Not Use" tag and remove from service immediately. Report to supervisor for corrective action.
8 Restore Area and Sign Off. Ensure that the gate is fully functional and safe before removing LOTO. Sign and date the inspection log. File the log in the department’s safety records (retain for minimum 3 years as per WorkSafeBC requirements).
