Per NFPA 70E Section 130.5, an arc flash risk assessment must be performed before any person approaches exposed energized electrical conductors or circuit parts. This template documents the calculated arc flash boundary distance (typically 4 ft – 20 ft depending on voltage and fault current), the incident energy exposure level (cal/cm²), and the corresponding PPE category required. It also enforces the limited, restricted, and prohibited approach boundaries for shock protection as defined by NFPA 70E Table 130.4(D)(a). Proper completion of this form ensures compliance with OSHA 29 CFR 1910.333 and reduces liability in the event of an electrical incident.
- Equipment de-energized and confirmed zero-energy state via certified voltmeter prior to any work within the restricted approach boundary.
- Arc-rated clothing, face shield, hearing protection, rubber insulating gloves with leather protectors, and voltage-rated tools — all must be inspected and within 12-month test date.
- Lockout/tagout applied in accordance with CSA Z460-20 and written energy control procedures verified.
- Arc flash boundary and shock approach boundaries (limited, restricted, prohibited) must be computed and posted on the equipment label per NFPA 70E 130.5(H).
- All personnel performing the assessment must hold valid NFPA 70E electrical safety training (refresher within last 12 months).
