Portable Ladder Inspection And Three Point Contact Setup

This procedure defines the mandatory inspection process for all portable ladders (step, extension, and platform) used on site, and the correct three-point contact climbing technique to reduce fall risk. Applies to fibreglass, aluminium, and wooden ladders rated for industrial use.

Purpose

The purpose of this SOP is to establish a consistent, verifiable inspection routine for all portable ladders and to standardise the three-point contact climbing method. By following these steps, operators will minimise the risk of ladder failure and fall-related injuries, ensuring compliance with CSA Z11 and WorkSafeBC regulations.

Scope

This SOP applies to all portable ladders used in maintenance, construction, and general industrial operations at Ryxen client sites. It covers step ladders, extension ladders, and platform ladders. This document does not cover fixed ladders, articulated ladders, or mobile scaffolding towers. All personnel who climb a portable ladder at a height exceeding 3 metres (10 feet) must be trained on this procedure.

Safety Precautions
  • LOTOUse ladder at least 1 metre away from electrical lines. For work near live circuits, use fibreglass ladders only. No metal ladders within 3 metres of exposed energised parts.
  • PPEWear CSA‑approved hard hat, slip‑resistant work boots, and high‑visibility vest when climbing ladders above 2.5 metres. Gloves must allow tactile grip on rungs.
  • FALLNever climb a ladder if you are dizzy, fatigued, or taking medication that impairs balance. Maximum weight rating (user + tools) must not exceed the ladder's duty rating (Type IA recommended).
  • HANDMaintain three points of contact at all times. Never carry objects in your hands while climbing – use a tool belt, lanyard, or hoist line. Keep your body centred between the side rails.

  • Rubber ladder feet / leg levelers (as needed for uneven ground)

Procedure: Step-by-Step
1 Pre‑inspection – check ladder condition. Visually inspect the entire ladder for cracks, bent rungs, loose rivets, corrosion, or any damage. Check that all spreaders and locking mechanisms operate freely. If damage is found, tag the ladder as DO NOT USE and remove from service.
2 Set ladder base on stable, level ground. Place the ladder on a firm, level surface. Use leg levelers if available. Ensure the area is clear of debris, water, or obstructions that could cause slipping. If the ground is soft, use a ladder stabiliser or base plate.
3 Establish proper ladder angle (4:1 rule). Angle the ladder so that the base is one foot away from the wall for every four feet of ladder height. The base should be set at a 75.5° angle. Use a spirit level to confirm – the bubble should be centred.
4 Secure the top and bottom. Engage all locking devices. If using extension ladders, verify the rung locks are fully engaged. Tie off the ladder at the top or bottom using straps to prevent side‑slip or walking. For step ladders, ensure the spreader bars are locked.
5 Three‑point contact climb method. Face the ladder at all times. Maintain three points of contact (two hands and one foot, or two feet and one hand) on the ladder while climbing or descending. Never carry tools in your hands – use a tool belt or hoist. Ascend/descend slowly, one rung at a time.
6 Work positioning. Once at the working height, keep your body centred between the ladder rails. Do not overreach – if you can't reach comfortably, descend and reposition the ladder. Never lean while standing on a rung. Maximum reach: keep your belt buckle between the rails.
7 Post‑use inspection and storage. Lower the ladder, inspect again for any damage incurred during use. Clean off mud, oil, or debris. If the ladder shows signs of impact or stress, tag it for maintenance disposal. Store the ladder in a dry, well‑ventilated area away from extreme temperatures. Hang it horizontally on dedicated racks, never lean against walls.
