Overall Equipment Effectiveness OEE Shift Tracking Log

This log records the three core OEE metrics — Availability, Performance, and Quality — for each production shift. Use it to calculate overall equipment effectiveness, identify losses, and drive continuous improvement initiatives.

Overview & Purpose

The OEE Shift Tracking Log captures the three fundamental components of Overall Equipment Effectiveness during each shift: Availability (planned vs. actual run time), Performance (ideal cycle time vs. actual cycle time), and Quality (good parts vs. total produced). By logging downtime events, cycle counts, and defect quantities, teams can calculate the true OEE percentage and prioritize loss categories (unplanned stops, minor stops, speed losses, defects, and startup losses). This form aligns with the OEE standard (ISO 22400) and is designed for printed clipboard use or digital entry into a CMMS.

Equipment & Safety Prerequisites
  • LOTOLockout/Tagout required before any manual adjustments, tool changes, or clearing jams during the shift.
  • DATAEnsure shift data collection tools (stopwatch, counter, data sheet) are available. Verify that the machine's cycle counter is zeroed at shift start.
  • PITPersonal protective equipment (PPE) per area: safety glasses, hearing protection, steel‑toed boots.

OEE Shift Inspection / Log Matrix

Status (P/F) Inspection Item / Task Tolerance / Threshold Actual Reading
[ ] Pass [ ] Fail Availability: Total planned production time (shift mins minus planned breaks). Record actual run time (planned time minus unplanned downtime). Availability % = (Run Time / Planned Time) × 100 ≥ 90% Planned: _____ min
Run: _____ min
Unplanned downtime: _____ min
[ ] Pass [ ] Fail Performance: Count total pieces produced during run time. Compare to ideal cycle rate (parts per hour). Performance % = (Ideal Cycle Time × Total Parts) / Run Time × 100 ≥ 95% Ideal cycle: _____ sec/part
Total parts: _____
Run time: _____ min
[ ] Pass [ ] Fail Quality: Count good parts (sold or passed QA) vs. total produced. Defects include scrap and rework. Quality % = (Good Parts / Total Parts) × 100 ≥ 97% Good: _____
Defects: _____
Total: _____
[ ] Pass [ ] Fail Shift Changeover: Verify previous shift handoff documentation is complete. Handoff form signed and data reconciled within 15 min Completed: _____ (Y/N)
[ ] Pass [ ] Fail Downtime Log: Record each unplanned stop event (duration + reason code). All stops > 2 min must be logged with reason # events: _____
Total STD: _____ min
