Policy Overview

This policy defines the mandatory documentation procedures to be followed immediately after any fleet vehicle accident. Every driver, supervisor, and fleet manager must adhere to the scene documentation template to preserve evidence, record witness statements, capture vehicle and road conditions, and complete all regulatory incident reports. Failure to follow this protocol exposes the company to liability, delayed insurance claims, and DOT non-compliance penalties. This template standardises the five critical documentation phases: scene safety, evidence capture, witness management, digital reporting, and chain-of-custody for retained materials.

Scope & Applicability

This policy applies to all company-owned, leased, and rental vehicles operated under the corporate fleet registration, including light-duty trucks, heavy commercial vehicles, trailers, and service vans. It covers all drivers (full-time, part-time, seasonal, and contractors when operating company equipment) and any incident involving property damage, personal injury, fatality, rollaway, or environmental spill. The documentation template must be used for every accident, regardless of severity, and the completed report must be submitted to Fleet Operations within 4 hours of the incident. Third-party vehicles involved in collisions with fleet assets also fall under this scope.

Core Directives & Compliance Rules
  • S1Scene Security: Immediately assess for hazards (fuel leaks, unstable loads, traffic). Activate hazard lights, set reflective triangles, and move to a safe location if possible. Do not disturb evidence prior to documentation.
  • S2Digital Evidence Capture: Use the approved mobile app or camera to photograph the entire scene from at least four angles (overview, approach, impact zones, final resting positions). Capture damage close-ups, tire marks, debris, road signs, weather conditions, and all involved vehicle license plates.
  • S3Witness & Driver Statements: Collect full contact information (name, phone, email, insurance details) from all involved parties and independent witnesses. Record verbatim verbal accounts using audio capture (with consent). Every driver must complete the Accident Scene Narrative Form within 20 minutes.
  • S4Regulatory Reporting: Comply with all jurisdictional reporting thresholds (police notification for injuries/property damage over $5,000 CAD, provincial trucking authority reporting for commercial vehicles). Submit internal incident report via Fleet Dashboard no later than 2 hours post-incident.
Roles & Responsibilities
  • Driver (Primary): Ensure personal safety, secure the scene, collect initial evidence, obtain witness data, and complete the Accident Scene Narrative Form. Never admit fault. Notify Fleet Dispatch immediately.
  • Fleet Dispatch / Supervisor (On-Call): Receive initial notification, dispatch a secondary response unit if required, assist with police coordination, and initiate the digital accident report log entry within the Fleet Operations system.
  • Fleet Safety Manager: Review all documentation within 24 hours, coordinate with legal/insurance adjusters, perform root-cause analysis using the scene evidence, and update the corporate accident database. Ensure any corrective actions are issued within 72 hours.
  • Claims & Compliance Administrator: Maintain the chain-of-custody for all physical evidence (photographs, video recordings, vehicle electronic data), upload completed reports to the central repository, and track regulatory filing deadlines.

Incident Reporting & Enforcement Steps
1 Immediate Scene Assessment (0–5 min): Driver performs a 360° safety scan, activates vehicle hazard lights, places emergency triangles, and calls 911 if injuries or hazards exist. Do not move vehicles until evidence is documented.
2 Evidence Collection (5–30 min): Using the company-approved smartphone app or camera, capture all required photos (minimum 20 angles), record a 360° video of the scene, note GPS coordinates, weather, and road surface conditions. Collect all driver licences and insurance cards.
3 Witness & Narrative (30–45 min): Complete the digital Accident Scene Narrative Form. Interview all witnesses and record their verbatim statements. Attach all photos, videos, and PDFs to the incident record.
4 Submission & Escalation (within 2 hours): Submit completed documentation package to Fleet Operations via the Fleet Dashboard. The Fleet Safety Manager receives auto-alert. For serious incidents (injury/fatality/criminal investigation), immediately escalate to the VP of Operations and Legal Counsel.
5 Post-Incident Review (within 72 hours): Fleet Safety Manager reviews all materials, leads a root-cause analysis meeting, assigns corrective actions, and updates the fleet accident database. Non-compliance with any documentation step results in mandatory re-training and potential disciplinary action.
