Purpose

To establish a uniform, annually repeatable procedure for evaluating the integrity and effectiveness of machine guarding systems. This audit ensures that every guard—fixed, interlocked, or presence-sensing—functions as engineered, that no unsafe gaps or modifications exist, and that compliance documentation is up to date. The ultimate goal is to prevent amputations, crush injuries, and caught-between incidents on industrial machinery.

Scope

This SOP applies to all production equipment within the facility that requires guarding under CSA Z432‑16 (Safeguarding of Machinery) and WorkSafeBC OHS Regulation Part 12. It covers fixed barrier guards, interlocked guards with or without locking, light curtains, pressure-sensitive mats, two-hand controls, and presence-sensing devices. Excluded are administrative controls and personal protective equipment (PPE) which are covered in separate SOPs. The audit is performed annually, or sooner following a near-miss, guard modification, or machine relocation.

Safety Precautions
  • LOTOLockout/tagout all energy sources to the machine before performing any physical inspection, gap measurement, or maintenance of guards. Verify zero energy state with a qualified electrician.
  • PPEWear CSA‑approved safety glasses with side shields, steel-toed boots, cut-resistant gloves (ANSI A3 or higher), and hearing protection when in active production zones.
  • ISOLATIONDo not reach or place any body part into the danger zone while testing interlocks. Use a non‑conductive test stick to actuate switches.
  • COMMSClearly communicate with the machine operator and supervisor before and during the audit. Post warning signage on the machine control panel.
Required Tools & Materials
  • Digital camera (minimum 12 MP) for photo evidence
  • ANSI/CSA Z432 reach‑distance probe set (gap gauges: 4 mm, 8 mm, 20 mm, 50 mm)
  • Non‑conductive test stick (1 m length) for actuating interlocks without entering the danger zone
  • Torque wrench with bit set (for verifying bolted guard fasteners to manufacturer spec)
  • Multimeter with continuity and switch test function (Fluke 87V or equivalent)
  • Last year’s audit report, machine guarding risk assessment, and manufacturer guard drawings
  • Red danger tags (LOTO‑type) and zip ties

Procedure: Step-by-Step
1 Review prior documentation. Retrieve the previous year’s guard audit report, corrective action log, and the machine’s current risk assessment. Note any open non‑conformances, modifications, or maintenance orders that may affect guarding.
2 Conduct visual inspection of all guards. With the machine locked out, examine each fixed, interlocked, and adjustable guard for cracks, deformation, corrosion, missing or loose fasteners (bolts, hinges, latches), and evidence of unauthorized removal or bypass. Document with photos.
3 Test interlock systems. Restore power only for interlock testing, following safe reset procedures. Open each guard while the machine is running in safe mode (low speed, no material). Verify the machine stops within the manufacturer‑specified time (typically ≤ 100 msec). Test that the machine cannot restart until the guard is closed and latched. Repeat 3 times per interlock.
4 Verify presence‑sensing devices. For light curtains, confirm alignment and that the minimum object sensitivity (MOS) matches the application. Use the test piece (e.g., 14 mm rod for hand protection) to block each beam; the machine must stop. Do the same for pressure mats: step on mat with required weight (typically 30 kg) and confirm immediate stop. Record response time.
5 Measure gap compliance. Using the CSA Z432 reach‑distance probe set, measure all openings in fixed guards. Ensure no gap exceeds the distance‑to‑hazard table (e.g., ≤ 4 mm for hazards within 50 mm; ≤ 20 mm for hazards beyond 850 mm). Record any non‑compliant gap and flag for immediate correction.
6 Document findings and tag deficiencies. Complete the digital or paper audit checklist. Any guard that is missing, damaged, or bypassed shall be tagged with a red danger tag at the machine control panel. Take additional photos of the tagged deficiency. Enter all findings into the CMMS or compliance log.
7 Review with maintenance manager. Present the completed audit, highlighting critical deficiencies that require immediate lockout and repair. Prioritize corrective actions: severity, risk level, and cost. Assign responsible parties and target completion dates. Update the risk assessment if needed.
