Overview & Purpose

This audit checklist ensures all loading dock equipment restraint systems — including mechanical and hydraulic dock lock mechanisms, trailer hook restraints, wheel chocks, and interlock warning lights — are fully operational before any trailer is loaded or unloaded. Compliance with OSHA 1910.178, DOT Part 393, and local CVOR carrier safety standards is verified at every active dock position. Each inspection line captures a pass/fail/not-applicable status with space for defect notes and immediate containment actions.

Prerequisites & Context
  • REQValid dock safety training certificate for all personnel performing inspections (e.g., Forklift & Dock Safety, OSHA 10/30).
  • REQTrailer must be parked with wheels chocked, engine off, and brakes engaged before dock restraint inspection begins.
  • INFORed / green light communication system must be tested at every dock position before first trailer is spotted.
  • INFOAny equipment failing inspection must be tagged out of service immediately; notify the fleet safety supervisor.

Audit / Inspection Matrix

Check / Task Status (P/F/NA) Requirement / Threshold Notes / Defect Details
Vehicle restraint hook — visual & functional test [ ] Hook engages trailer ICC bar fully; no cracks, deformation, or weld failure. Auto-lock functions. _________________
Wheel chocks — condition & placement [ ] Rubber/urethane chock with good tread; placed snugly against rear trailer wheels both sides. _________________
Dock leveler — hydraulic/mechanical operation [ ] Leveler rises fully, extends lip, and holds under load. No fluid leaks (hydraulic) or spring fatigue. _________________
Interlock light system — dock & exterior signals [ ] Red light inside = restraint NOT engaged / do not load. Green light inside = restraint engaged / safe. _________________
Trailer ICC bar — structural integrity [ ] No bends, cracks, or rust-through. Bar height within 30–60 cm (12–24 in) from ground per DOT spec. _________________
Communication system — dock phone / pressure bell [ ] Two-way communication operational between driver and dock worker; audible and visual signals. _________________
Edge-of-dock guardrails & safety gates [ ] Gates swing freely, latch securely. No broken welds, missing bolts, or corrosion on anchor points. _________________
Dock seal / shelter — clearance & trailer contact [ ] Seal compresses evenly around trailer opening; no tears or exposed gaps that could cause temperature loss. _________________
